This is a very comprehensive, explicit and clear way of peer review! It's also very user friendly and pretty easy to understand. I think that this will be able to elicit good, constructive comments. However, it is slightly long which may deter peer reviewers from putting forth the effort to complete it well with good thought.

I've used something just like this for my ESL academic writing class. It works really well.  :D Like Tracy said, it is quite long, but when you give students enough time I find they're happy to finish it. Helpful to both student reviewer and student writer.
